- My signature on this form constitutes a “signature on file” and is agreement to pay the charges indicated, which will be debited / charged to the designated account I have provided.
- The amount debited / charged may be reflected in my account balance prior to loading or unloading.
- I am legally obligated to pay for all transportation and related charges in accordance with the rules and regulations of the U.S. Department of Transportation.
- I agree to notify Findlay International, Inc. in writing of any changes in my account information or termination of this authorization fifteen (15) days prior to the next due date of the charges.
- For ACH debits to my checking/saving account, I acknowledge that the origination of ACH transactions to my account must comply with the provisions of U.S. law.
- We may apply a fee to cover all or part of our costs of accepting your credit card.
